Job Title: Scheduling Coordinator

Summary:
We're looking for a highly organized and detail-oriented Scheduling Coordinator to provide essential after-hours scheduling and operational support to our field nursing and clinical teams. This remote position is crucial for ensuring the timely and efficient scheduling of patient visits, enabling our clinicians to optimize their patient interactions and deliver high-quality home healthcare. You'll play a direct role in the seamless delivery of care by expertly managing visit logistics and supporting our field staff.
Responsibilities:
- Assign visits and complete associated scheduling tasks within the HomeCare HomeBase (HCHB) EMR system to support field clinicians.
- Monitor EMR workflows to ensure timely documentation and progress of patient care tasks related to scheduling.
- Prioritize and triage incoming scheduling requests based on urgency and patient care needs.
- Effectively communicate and collaborate with triage and field clinicians regarding visit assignments and schedule adjustments.
- Demonstrate the ability to work autonomously in an organized work environment, managing a dynamic scheduling caseload.
- Respond to user requests for scheduling assistance, completing tasks timely, accurately, and efficiently.
- Escalate unresolved or critical scheduling issues to leadership when appropriate.
- Maintain productivity standards and participate in meetings or training sessions.
- Demonstrate reliable attendance and adherence to assigned shifts.
- Candidates must be available to work weekends and holidays, as scheduling needs dictate.

Qualifications:
- High School Diploma or equivalent required.
- Minimum 1 year of professional experience in healthcare administration, patient care, or home health/hospice care, with a strong emphasis on scheduling.
- Experience using HomeCare HomeBase (HCHB) EMR strongly preferred.
- Experience in a professional healthcare setting, including remote roles.
- Exceptional organizational skills and meticulous attention to detail for managing complex schedules.
- Strong problem-solving and critical thinking abilities, particularly in resolving scheduling conflicts.
- Excellent communication and customer service skills, especially when interacting with clinicians and addressing scheduling needs.
- High level of accuracy in data entry related to patient appointments and clinician assignments.
- Comfortable with fast-paced, remote work environments and able to meet deadlines effectively.
- Proficient in multitasking across systems and managing real-time scheduling requests.
- Working knowledge of medical terminology.
- Proficient in Microsoft Office applications (Word, Excel, Teams, Outlook).
- Excellent telephone etiquette.
- Must meet CareXM's current IT Computer and Internet requirements.
